$20,000.00 Gift for Cancer Treatments Made to Foundation by Grateful Client

The Alabama Veterinary Medical Foundation is pleased to announce a large contribution designated for cancer treatment of pets owned by financially needy clients. The treatment of these patients may be performed at your clinic or in a referral institution and will apply to all treatments or surgeries related to the cancer diagnosis. Please express thanks to Dr. Arthur Moore of Headland, who solicited the gift from one of his clients. The provisions of this grant are as follows:
1. The gift will be used to provide cancer care grants for pets of owners who are able to demonstrate true financial need and that all credit options have been exhausted. The following parameters meet the definition of “Financially Needy”:
The client may be a Medicaid recipient (NOT MEDICARE). As with the Maddies fund applications, a copy of the Medicaid card and photo ID must be submitted with each application.

The Client must have been declined for Care Credit. A copy of the non-approval receipt must be submitted. (If you do not currently have Care Credit in your office your client must then be a Medicaid recipient.) Contact Care Credit for more information on their services. The client may apply for Care Credit at any office in the event you do not have the service in your clinic. Other comparable credit services to Care Credit may be considered. This does not include being declined by a major credit card such as Visa, MasterCard, Amex, Discover, Diners Club etc.
2. Pet owners receiving grants must be willing and financially able to pay
20% of the total treatment charge.
3. Individual grants will not exceed $4,000.00
4. You must be a member of the Alabama Veterinary Medical Association,
in good standing to apply for a grant.
5. The pet must have a good prognosis for survival upon completion of the
oncology treatment.
